Session Summary
Using a case-based format, our panel will work through a challenging polypharmacy scenario, highlighting the complexities of medication management in LTC. Through discussion and audience engagement, the session will emphasize collaborative approaches to care, with a focus on optimizing outcomes for residents.
The discussion will explore:
- Interdisciplinary approaches to complex medication management
- De-prescribing in the context of frailty and multimorbidity
- Balancing clinical guidelines with resident goals of care
- Practical strategies for collaboration across care teams
- Common challenges and clinical pearls from each discipline
