Session Summary

Using a case-based format, our panel will work through a challenging polypharmacy scenario, highlighting the complexities of medication management in LTC. Through discussion and audience engagement, the session will emphasize collaborative approaches to care, with a focus on optimizing outcomes for residents.

The discussion will explore:

  1. Interdisciplinary approaches to complex medication management
  2. De-prescribing in the context of frailty and multimorbidity
  3. Balancing clinical guidelines with resident goals of care
  4. Practical strategies for collaboration across care teams
  5. Common challenges and clinical pearls from each discipline

This one-credit per hour Group Learning program has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit.

To be eligible for credit: Participants must attend the full session. A mandatory post-session survey must be completed to receive your certificate.
