As a Medical Director you will work closely and collaboratively with the Executive Director of the home as well as the clinical leadership team, including the Director of Care, to lead, implement, and evaluate medical services. As an Attending Physician, you will provide high quality resident-centred medical care as a member of the care team. There are 47 residents at Anson Place. 

Responsibilities

  • Provides leadership over medical services provided in the homes.

  • Willing to provide clinical care to assigned residents, including in-person weekly rounding.

  • Regularly communicates with the Director of Care regarding relevant policies, procedures, and responsibilities.

  • Provides on-call coverage for all residents, after hours, in accordance with the LTC on-call program as established between the Ministry of Health and Long-Term Care (MOLTC) and the Ontario Medical Association (OMA)

  • Is a strong advocate for interdisciplinary resident-family integrated care and participates in quality improvement activities in collaboration with home’s leadership, interdisciplinary team, with residents and their families and caregivers, and with health system partners.

  • Addresses resident acute and chronic care needs and provides appropriate and timely documentation in electronic medical records (EMR) systems.

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team.

  • Identifies circumstances in which residents require referral to the appropriate medical consultant and/or other health practitioner and facilitates this care, participating in care conferences and communications.

  • Conducts regular medication drug review and documents/updates resident’s health record.

  • Commits to continuing education related to long term care, care of the elderly, palliative care, and dementia care.

QUALIFICATIONS

  • Medical practitioner licensed to practice medicine in the Province of Ontario, and a member in good standing of the Colleges of Physicians and Surgeons of Ontario (CPSO)

  • Experience working in Long Term Care (LTC) as an Attending Physician, or preferably, as a Medical Director

  • Must complete the OLTCC Medical Director course within 12 months of assuming the position of Medical Director

  • Ability to build trust within the home leadership team, interprofessional care team, and residents and their families and caregivers, through meaningful interactions and excellent medical care

  • Ability to adapt, provide coaching, facilitate change, and continually improve resident care

  • Admitting privileges at a local acute care hospital, or has specified alternate arrangements for acute hospital admission if required, preferred

  • Knowledge of regulations governing Long Term Care

  • Ability to work effectively as part of an interdisciplinary team

Attending Physician

Good Samaritan Seniors’ Living, Long-Term Care Home has been a landmark in Alliston, Ontario since 1974. Known affectionately by many as the “Good Sam” we are home to 64 Residents in our Long-Term Care Home.

We are currently recruiting an Attending Physician to join our team. This rewarding role is perfect for someone who is interested in providing medical care/support to Residents in the Long-Term Care Home. Working with our Medical Director, Dr. Brian Swarbreck, you will provide a weekly visit to the Home and coordinate after-hours/on-call services with Dr. Swarbreck for weekends/holidays/vacation.

Attending Physician

The Attending Physician position will be located at Extendicare Crossing Bridge, our new State-of-the-Art 256 bed home located in Stittsville, Ontario.

Reporting to the Medical Director, the Attending Physician will provide high quality resident centred medical care as a member of the management care team. The resident roster will be approximately 64.

ACCOUNTABILITIES

  • Attends the long-term care home on a weekly basis, to provide clinical care to assigned residents and clients

  • Addresses resident acute and chronic care needs and provides appropriate and timely documentation in Extendicare’s electronic medical records systems

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team

  • Identifies circumstances in which residents require referral to the appropriate medical consultant and/or other health practitioner and facilitate this care, participating in care conferences and communications.

  • Works effectively and efficiently with members of the interdisciplinary team including the medical director to ensure the highest level of care is provided to residents

  • Conducts regular medication drug review and documents/updates resident’s health record

  • Participates in after hour on-call

  • Commits to continuing education related to long term care, care of the elderly, palliative care, and dementia care.

  • Participates in physician peer review to continually improve resident care.

QUALIFICATIONS

  • Medical practitioner license to practice medicine in the Province of Ontario, and be a member in good standing of the Colleges of Physicians and Surgeons of Ontario (CPSO)

Medical Director and / or Attending Physician

The Medical Director will be located at Summit Place in Owen Sound, Ontario.

Reporting to the Administrator, the Medical Director will work closely with the clinical leadership team, including the Director of Care, to collaboratively lead, implement, and evaluate medical services. As Attending Physician, s/he will provide high quality resident-centred medical care as a member of the care team. The resident roster will be approximately 40 residents (caseload may be negotiable).

There is also the option of applying for Attending Physician only, without Medical Director responsibilities. The resident roster will be approximately 40 residents with this position.

Accountabilities

  • Provides leadership over medical services provided in the homes.

  • Willing to provide clinical care to assigned residents, including in-person weekly rounding

  • Regularly communicates with other Attending Physicians and / or Nurse Practitioners regarding relevant policies, procedures, and responsibilities

  • Ensures appropriate on-call coverage has been arranged for all residents, after hours, in accordance with the LTC on-call program as established between the Ministry of Health and Long-Term Care (MOLTC) and the Ontario Medical Association (OMA)

  • Is a strong advocate for interdisciplinary resident-family integrated care and participates in quality improvement activities in collaboration with home’s leadership, interdisciplinary team, with residents and their families and caregivers, and with health system partners.

  • In partnership with the Chief Medical Officer (Extendicare) and home leadership develops quality of care standards for home and fosters a physician culture that includes objective setting, peer comparison, audit and feedback, quality improvement plans, and annual activity / performance review

  • Addresses resident acute and chronic care needs and provides appropriate and timely documentation in Extendicare’s electronic medical records (EMR) systems

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team

  • Identifies circumstances in which residents require referral to the appropriate medical consultant and/or other health practitioner and facilitates this care, participating in care conferences and communications.

  • Conducts regular medication drug review and documents/updates resident’s health record

Attending Physician at Muskoka Landing

As a proudly Canadian organization, with 14 Long-Term Care (LTC) Homes and 7 Retirement Lodges across Ontario, Jarlette Health Services has established itself as an industry leader with over 50 years of experience in the dynamic senior care sector.

Muskoka Landing is a fully accredited long term-care home in Huntsville, Ontario. Our home has a full time Nurse Practitioner, Medical Director and two Attending Physicians. We are looking for a third Attending Physician. At Muskoka Landing we have 94 long-term beds available and offer private and basic accommodations in a brightly lit, friendly home. Muskoka Landing contains beautifully landscaped gardens and walkways overlooking an unrivaled view of Fairy Lake.

Position Summary:

The Attending Physician reports to and works collaboratively with the Administrator and Medical Director. The Attending Physician would be responsible for providing exemplary resident-centered medical care and would support a roster of approximately 25 to 32 residents.

Required Traits and Qualifications:

  • Medical practitioner licensed to practice medicine in the Province of Ontario, and a member in good standing of the Colleges of Physicians and Surgeons of Ontario (CPSO)

  • Experience working in LTC or willingness to have mentoring with our Medical Director and NP

  • Ability to build trust and interact in a professional manner with residents and their families as well as team members

  • Highly developed interpersonal skills and demonstrated ability to work effectively as part of a team

    Desirable qualifications / experience (not required):

  • Knowledge of legislations regulations governing LTC (e.g. Fixing Long-Term Act 2021 and Ontario Regulation 246/22)

  • Experience with PointClickCare, eMar, eConnect

  • Knowledge of MDS and Resident Care Plan review/updating

    Accountabilities:

  • Attends weekly in person rounds in the LTC home to provide acute clinical care, preventative assessments, and improve quality of life for those with chronic conditions;

  • Must record a SOAP note in our PointClickCare electronic health record to document your visit;

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team;

  • Works effectively and efficiently with members of the interdisciplinary team including the Medical Director to ensure the highest level of care is provided to residents;

  • Conducts regular medication drug review every 3 months with pharmacist;

  • Participation in the after hour on-call rotation that is only 17:00 - 22:00 Monday to Friday for Attending Physicians currently as our Medical Director covers the other times.
