Through discussion and audience engagement, the session will emphasize collaborative approaches to care, with a focus on optimizing outcomes for residents.\nThe discussion will explore:\n\nInterdisciplinary approaches to complex medication management\nDe-prescribing in the context of frailty and multimorbidity\nBalancing clinical guidelines with resident goals of care\nPractical strategies for collaboration across care teams\nCommon challenges and clinical pearls from each discipline\n\n \nCOP is now accredited!\nThis one-credit per hour Group Learning program has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit.\nTo be eligible for credit: Participants must attend the full session.
