DESCRIPTION:Session Summary\nThis session will discuss quality improvement in the LTC setting including the widely used Model for Improvement framework and how clinicians can get involved and make an impact, within the regulatory frameworks and as champions and leaders within interprofessional teams.\n\nPresenters\nBenoit Robert, MD, MBA, Chief Medical Officer Perley Health and Rachel Ozer, PhD, MHA, Senior Knowledge Broker Ontario CLRI at Bruyère Health\n \nObjectives\n\nBe able to describe the dimensions of quality, the systemic approach of quality improvement compared to individual clinical care, and the Model for Improvement used widely in healthcare to set overall goals and conduct iterative tests of change for sustainable improvement\nUnderstand the annual quality improvement process in LTC homes, common current objectives and opportunities to align clinical priorities and concerns with organizational efforts\n\nBe able to recognize the components of teamwork in LTC practice and opportunities to be an influencer for change through individual contributions as well as collaborative leadership\n\n\nUnderstand the quality improvement practice requirements on physicians and how these could be applied to clinical practice in LTC\n\n\nCOP is now accredited!\nThis one-credit per hour Group Learning program has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 1 Mainpro+ credit.\nTo be eligible for credit:\nParticipants must remain available for the entire duration of the session.  A mandatory post-session survey must be completed in order to receive your certificate.\nRegistration is required to attend.
